Optically clear cytoplasm can present a diagnostic dilemma in various organ systems, particularly in the head and neck region. This review focuses on clear cell changes in salivary, odontogenic, thyroid, parathyroid, or sinonasal/skull base lesions, as well as metastases to these regions.
