His commitment to education\, innovation\, and community service—including pro bono surgical care for underserved populations—has cemented his legacy in maxillofacial surgery. \n\n\n\n\n\n\n\nAbout the Lecture: \n\n\n\nBone has a significant capacity for repair and regeneration following injury or surgery\, involving complex interactions between cells\, growth factors\, and the extracellular matrix. Repair restores tissue continuity without increasing bone volume\, while regeneration generates new bone\, increasing skeletal tissue volume. Distraction Osteogenesis( DO) is a controlled surgical technique that uses mechanical strain to stimulate bone regeneration. It is used to treat post-surgical defects\, non-healing fractures\, osteomyelitis\, alveolar bone loss\, and congenital skeletal deformities. Maxillo-mandibular defects resulting from tumor resection present profound challenges in both function and aesthetics\, especially in the post-chemo-irradiation phase. Traditional reconstructive techniques\, such as revascularized free fibula flaps (RFFF)\, remain the gold standard but have limitations\, particularly in replicating the natural curvature of the mandible\, maxilla and palatal vault. The advent of Curvilinear Transport Distraction Osteogenesis (CTDO) has revolutionized Maxillo-mandibular reconstruction by enabling controlled bone regeneration along a curved trajectory\, restoring both form and function.  \n\n\n\nCTDO builds on the principles of DO  but innovates through tetrafocal distraction and hybridized transport disk technology. This technique overcomes challenges posed by traditional RFFF\, including poor alveolar height\, compromised vestibular depth\, and inadequate peri-implant support. By eliminating the “rubber band effect\,” which previously restricted bone regeneration to linear paths\, CTDO ensures predictable and stable outcomes. Clinical and radiological evaluations confirm that this approach provides a reliable alternative for complex maxilla-mandibular reconstructions\, yielding high-quality bone with adequate density for implant placement. This presentation will highlight the biological basis\, surgical methodology\, and clinical outcomes of CTDO\, demonstrating its superiority over conventional techniques. By integrating biomechanics\, regenerative principles\, and advanced surgical engineering\, CTDO enhances patient outcomes and expands the reconstructive possibilities in oral and maxillofacial surgery\, improving the quality of life for patients requiring extensive Maxillo-mandibular rehabilitation.
